What Is PTSD?

Post-traumatic stress disorder (PTSD) is a disabling psychiatric condition that develops after exposure to a traumatic event — either directly experiencing it, witnessing it, learning that it happened to a close family member or friend, or experiencing repeated or extreme exposure to aversive details of traumatic events. It is characterized by four distinct symptom categories: intrusions (unwanted memories, nightmares, and flashbacks), avoidance of trauma reminders, negative alterations in cognition and mood, and alterations in arousal and reactivity (hypervigilance, sleep disturbance, irritability).

According to the DSM-5-TR, symptoms must persist for more than 1 month after the trauma and cause clinically significant distress or impairment in social, occupational, or other important areas of functioning. Globally, an estimated 3.9% of the world population has had PTSD at some stage in their lives. While approximately 70% of people experience a potentially traumatic event during their lifetime, only a minority (5.6%) go on to develop PTSD. Rates are more than three times higher (15.3%) among people exposed to violent conflict or war, and especially high following sexual violence.

The good news: PTSD is highly treatable. With trauma-focused psychotherapy (such as Prolonged Exposure or Cognitive Processing Therapy) and/or SSRI medication management, most people experience meaningful relief — and a return to the life they deserve.

Acute stress disorder occurs in the first month after a traumatic event and can only be diagnosed within that window. PTSD can only be diagnosed at least one month after the trauma. Acute stress disorder can develop directly into PTSD, or PTSD can develop months or even years after the trauma without preceding problems being obvious. The symptoms are similar — intrusions, avoidance, negative mood, and hyperarousal — but the timing distinguishes them. If you’re experiencing symptoms that have persisted beyond a month, Dr. Vuchansu can provide a comprehensive evaluation to determine the correct diagnosis.
Not always. Trauma-focused psychotherapy (such as Prolonged Exposure or Cognitive Processing Therapy) is the first-line treatment for most adults with PTSD. SSRIs and SNRIs are reasonable alternatives and are preferred for patients with comorbid depression or anxiety disorders. For sleep disturbance and trauma-related nightmares, prazosin may be recommended. Dr. Vuchansu will evaluate your situation and discuss all options with you — no decision is made without your full understanding and consent.
