What Is Major Depressive Disorder?

Major depressive disorder (MDD) is more than feeling sad or having a bad week. It is a serious medical condition that affects how you think, feel, and function in daily life. To meet the diagnostic criteria for MDD, at least five symptoms must be present during the same 2-week period — and at least one must be either depressed mood or loss of interest or pleasure. A rough patch is usually temporary and linked to a specific situation. Major depressive disorder involves at least five symptoms lasting two weeks or more — including either depressed mood or loss of interest or pleasure — and causes real impairment in work, relationships, or daily functioning. If you’re unsure, an evaluation can give you clarity.
It depends on severity. For mild depression, psychotherapy alone is often recommended first — especially CBT, behavioral activation, or interpersonal therapy. For moderate to severe depression, combining medication with therapy is typically more effective than either alone. Dr. Vuchansu will discuss all options with you and build a plan that fits your needs.
Many people notice some improvement within 1 to 3 weeks, but full effects often take 6 to 8 weeks. Dr. Vuchansu typically schedules a follow-up within 2–4 weeks of starting medication to assess your response and make adjustments if needed. Early improvement — even 20% symptom reduction by week 4 — is a good sign that the medication is on the right track.
For most people, medication is continued for at least 6 to 12 months after symptoms improve to reduce the risk of relapse. If you’ve had multiple episodes, a longer duration may be recommended. Dr. Vuchansu will review your progress regularly and discuss any changes — including a gradual taper — when the time is right.
